We do not have a final full cost for this event yet, but in order to plan ahead efficiently we need to get deposits in for this trip. * You must have a current North Jax Baptist Medical Waiver form on file in order to participate in this event. If you have not submitted one for 2022, please complete the form attached to your confirmation email. Be sure to complete the entire section for Insurance information and have two witness signatures at the bottom of the form.
